RED BARONS 2011 FOOTBALL REGISTRATION

REGISTRATION IS ONLY AVAILABLE AT KERBY FIELD HOUSE
UNTIL FRIDAY, AUGUST 19TH, 2011.

You'll need the following information to register at the Field House:

1. Payment Type: Credit Card or Check (check must be sent to the P.O. Box below)

Payment for Red Barons is $250.00 for players and $115.00 for cheerleaders.
A 20% fee reduction applies for subsequent player/cheer within families.
Oldest child pays full price with next youngest at 20% off.

2. Insurance information.

3. Actual weight of football players.
4. Emergency contact information.

After You've Completed the Payment Process
You'll need to send the following documents to the Red Barons P.O. Box below:

1.      A signed ESFL Participant Form - 2011 ESFL Participant Form.
2.      A copy of child's physical exam from physician completed after January 1, 2011.
3.      Two (2) copies of registrant's birth certificate.

NOTE: Physical must be handed in no later than equipment handout. If all paperwork is not handed in by then, your player will not be able to begin practicing.

Retain your original, forms will not be returned.
